Primary and preventive care
Routine visits, physical exams, health screenings, and the ongoing relationship with a provider who knows your history.
Chronic condition management
Ongoing treatment and monitoring for conditions such as high blood pressure, diabetes, high cholesterol, and asthma.
Infectious disease care
Testing, treatment, and long-term management for HIV, hepatitis C, and sexually transmitted infections, including prevention with PrEP, PEP, and DoxyPEP.
Medication-assisted treatment
Medical treatment for opioid and other substance use disorders, provided together with counseling and recovery support.
Laboratory and on-site testing
Rapid testing and laboratory draws arranged through the network, with results reviewed by your provider and explained to you directly.
Care coordination
A care coordinator helps with referrals, records, transportation questions, benefits applications, and follow-up between visits.
One team, not five appointments in five places
Most people who come to us have been sent from one office to another and have given up somewhere along the way. We built our services to work the other direction. Your medical provider, your counselor, your care coordinator, and your pharmacist work from the same plan.
When we do not provide a service ourselves, we coordinate it through partner providers and stay involved until you are seen.

What to expect
Reach out
Call us or complete the intake packet online, and we will schedule your first appointment. You do not need a referral to become a patient.
First visit
We take a full history, review any medications you are taking, and talk through what you want out of your care.
Your plan
You leave with a written plan, any prescriptions or lab orders you need, and a scheduled follow-up.
Staying connected
Your care coordinator checks in, handles referrals, and helps you get back on track if you miss a visit.
What to bring to your first visit
- Photo identification, if you have one
- Insurance card, if you have coverage
- A list of your current medications, or the bottles themselves
- Records or test results from other providers, if you have them
- Questions you want to ask. Write them down before you come in.
If you do not have any of these, come anyway. We will work with what you have.
Insurance and cost
We see patients whether or not they have insurance. If you are uninsured, our staff will talk with you about your options before any service is provided, including assistance programs that may cover the cost of care or medications.
No one is turned away because of income, insurance status, immigration status, housing situation, or a past history with the health care system.
